We would like to know how many Christian students are attending high school at the Salt Lake area public high schools. This information will help us determine how best to provide campus ministry in the Salt Lake area. Absolutely no personal information is needed regarding individual students.

Basic Instructions: Items marked with "*" are required to complete the form. There are a total of 10 lines for you to enter school information. If you have students at 2 high schools, complete 2 lines; if you have students at 7 high schools, complete 7 lines and so on. Simply leave any unused school lines empty. After you have completed all required items simply click "SUBMIT" and your information will be sent.
